Death Gen Bipin Rawat: A Parliamentary Panel Report Indicates “Human Error” Caused Dec. 8, 2021, Chopper Crash

On December 8, 2021, Gen Bipin Rawat, the nation’s first Chief of Defence Staff, perished in a Mi-17 V5 helicopter crash, and a parliamentary panel investigate determined that the cause of the catastrophe was “human error”.

Gen Rawat, his wife Madhulika Rawat, and multiple other armed forces personnel were killed when the military helicopter they were traveling in crashed nearby Coonoor in Tamil Nadu.

In a report delivered to Parliament on Tuesday, the Standing Committee on Defence provided details on the number of Indian Air Force aircraft crashes during the 13th Defence Plan period.

A total of 34 crashes occurred, with nine IAF aircraft mishaps in 2021-22 and eleven in 2018-19.

The report’s tabular data also included a “reason” column, which indicated the cause of the accidents during this time period, as well as the aircraft type, date, and accident.

The report described the aircraft as “Mi-17”, the date as “08.12.2021”, and the reason of the accident as “HE(A)” or “human error” for the 33rd disaster.

The military ministry informed the panel that 34 investigations into these mishaps had taken place during that time.

“The ministry additional confirmed that the recommendations by these Inquiry teams review the process, procedure, training, equipment, culture, operations, maintenance and administration holistic with intent to prevent recurrence of an accident,” according to the release.

According to the ministry, “all remedial measures outlined in the Chief of Air Staff remarks are binding and actioned.”Most have been done while a few are under implementation,” the statement said.
